- Download, print AND fill out any sheets that are applicable to your condition, and bring these to the clinic at the time of your appointment. These forms include:
- Have your insurance card when presenting to the clinic for the first time or if your insurance has changed since the last visit.
- Have your co-pay available when presenting to the clinic (if this is applicable).
- If your insurance company is an HMO or another "managed care" program (i.e., Cascade Comprehensive Care), you will need to contact your primary care physician for authorization to be seen for any new orthopaedic problem.
- If you have had x-rays or MRIs on the affected extremity or spine outside of Merle West Medical Center, Cascades East, or Klamath Orthopedic Clinic, then please personally bring them to the clinic with you.

- FOR WORKERS FILING A WORKER COMPENSATION CLAIM:
- If you are a patient who was injured on the job, or you are filing a Worker's Compensation claim, please fill out an 827 form at home or work and bring it to the clinic ONLY IF:
- This is the first report of injury or disease
- Your visit is for an aggravation of original injury
- You are changing attending physicians from your previous physician to the physicians of Klamath Orthopedic Clinic
